Customizing Pre-Made Forms

Enhance Inquiry, Application, Illness, Accident, Incident, and Medication Authorization Forms with Custom Fields

Kindertales offers pre-made forms such as Inquiry, Application, Illness, Accident, Incident, and Medication Authorization forms, which come with predefined fields designed to streamline common daycare processes. Customizing a pre-made form entails adding additional fields to better align with individual daycare requirements. Follow these steps to tailor pre-made forms to your specific needs:

  1. Click Settings in the top right corner of the screen
  2. To customize your Inquiry or Application forms
    1. Select CRM and navigate to the Customize Forms tab.
    2. Choose the form you wish to customize from the dropdown
  3. To customize your Illness, Accident, or Incident forms
    1. Select Forms and navigate to the IAI (Illness Accident Incident) tab
    2. Choose the form you wish to customize from the dropdown
  4. To customize your Medication Authorization form
    1. Select Forms and navigate to the Medication tab
  5. Drag and drop field types from the interface to add custom fields to your form
  6. Reorder fields as needed by clicking and dragging them within the editing area
  7. Utilize the Edit pencil icon to customize each field type further
  8. Save your changes, and the customized fields will appear when users fill out the form

    Creating Custom Forms with Form Builder

    Streamline Data Collection with Tailored Forms

    Kindertales provides a Form Builder tool for creating entirely custom forms to streamline data collection processes. Follow these steps to create and activate custom forms:

    1. Click Settings in the top right corner of the screen
    2. Select the Forms tab and navigate to the Form Builder
    3. The Select a Location drop-down selects the location at which you will use this form

      Custom forms created at a location level will only be usable for children at that location. Forms created at the corporate level will appear at all locations.

    4. Specify who has access to the form, whether it's teachers and/or parents.
    5. Select form structure options such as Header, Text Field, Checkbox Group, Date Field, Signature, and more:

      Header Name of form
      Text Field Allows for a question from you and provides families a field for a short answer
      Text Area Same as text but provided families with a larger response area (both can be made a required field)
      Checkbox Group Allows families to select multiple answers
      Date Field Provides a date field
      Signature Provides an area for them to sign
      Paragraph This is an area where you can put your message not required text info. Note: The paragraph tool includes a word-style text styling bar for formatting large blocks of text.
      Number Provides a field for them to provide their number
      Radio button Only allow families to select one option from the options
      Select Drop-down menu with options to select
    6. Save the form once it's built and ready for use
    7. Confirm form settings on the Form Control tab, including form enabling, teacher access, parent access, and update alerts.
      • Form Enabled – Check the box in this column if you would like to make this form that must be completed in your enrollment process
      • Teacher Access – Check the box in this column if you would like your staff to be able to access this form on the classroom app
      • Parent Access – Check the box in this column if you would like your parents to have access to this form, but it is not mandatory to complete
      • Update Alerts – Check the box in this column if you would like a notification to be sent to your dashboard whenever a family member makes a change to this form

    Kindertales Tips:

    • Ensure the first element of each custom form is a Header, indicating the form's title.
    • Avoid pasting images into custom forms as this feature is not supported.
    • Utilize profile tags for form fields marked with a green tag icon to report on them effectively in the Analytics section of Kindertales.
